Home Health services include healthcare management and clinical experts that will provide uncompromising levels of attention, customer service and integrity. Welcome Care’s team of licensed Home Health Aides will work with patients and physicians to develop a plan of care that will meet each individual’s specific needs. Medicaid, Medicare & Private Insurances are accepted.
Skilled Nursing/RNs perform patient evaluations, including monitoring and tracking vital signs, clinical procedures such as IV placement, phlebotomy, and administering medications. In the home, your nurse will monitor your progress and keep in contact with your physician to make sure that your treatment protocols are appropriate.

Nursing Services and Programs

Intravenous Medication Administration
Physicians order the type of infusion, duration of care, and prognosis and our nurses implement the plan for clients requiring treatment and intravenous medication at home. Skilled nurse’s visit as often as needed to teach, implement, clean, change, and adjust all types of IVs and access ports .
Infusion Therapy Administration
Physicians order mode of infusion, duration of care, and prognosis and our nurses implement the plan for clients requiring treatment and intravenous medication at home.
Wound Care
Nurses specializing in wound care visit can daily teach proper wound care and to ensure proper treatment and healing of all wounds.
Diabetes Management
Our experienced nurses provide help with all aspects of managing diabetes including: blood glucose monitoring, insulin administration and tracking, exercise plan, menu planning, education and more.
Chronic Illness Management
Nurses continually assess the client and routinely communicate with the client’s physician to provide the best care.
Alzheimer’s Care/Dementia Care
Our nurses work with the patient’s family to educate them on proper medication routine and to help create a comfortable environment including simple routines and visual cues as a positive substitute for diminishing mental processes.
Medication Management
Nurses assess, monitor and teach patients to ensure they are taking medications appropriately, and understand what they are taking and why. They can also teach the patients and families to set up medication boxes to ensure safety and compliance.

Physical Therapy

Physical Therapists perform home evaluations and determine appropriate treatment plans using techniques designed to increase mobility, reduce pain, restore function and prevent disability. Physical Therapy Services and Programs Include:

Speech Therapy

Speech Pathologists are responsible for the assessment and treatment of speech, language, and swallowing, cognitive and voice disorders. Through the use of multiple therapy techniques, therapists provide a well-rounded program of care to help remedy a speech or swallowing problem.

Occupational Therapy

Occupational Therapists help patients improve their ability to perform tasks in their home living environments. Occupational therapist use treatments to develop, recover, or maintain the daily living skills of their patients. After thorough assessments, therapists utilize adaptive equipment to help restore the client’s everyday living skills. We also offer family training to ensure a smooth transition back home.
