Getting started is simpler than most families expect. Most of the process happens through a handful of short conversations, not a stack of forms you have to figure out alone.
Call our office or fill out a short form. We ask a few basic questions upfront, like your child’s or loved one’s diagnosis and current needs, so your first real conversation isn’t spent on paperwork. This also gives our team a head start before we speak with you directly.
Next, we walk through what’s required for approval. This includes diagnosis records, evaluation reports, and sometimes school documentation depending on your case. Most families haven’t seen this full list before, and knowing it early saves time once you’re ready to submit. We flag anything that looks incomplete before it becomes a delay.
Once you’re enrolled, you’ll work with a Care Coordination Organization for ongoing support. We explain what your Care Manager actually does day to day, since most families have never worked with one before and aren’t sure what to expect. We stay available if questions come up after that first meeting.
Once services are approved, we schedule ABA therapy, PT, OT, speech therapy, or home health aide support based on what your family actually needs. Anyone with a documented intellectual or developmental disability that began before age 22 may qualify. This includes autism, cerebral palsy, Down syndrome, and other diagnoses. Our team can help you understand where your situation stands before you apply.
A CCO manages ongoing coordination between your medical, behavioral, and developmental disability services. You’ll be assigned a Care Manager who tracks your plan and connects you to approved providers. We help you understand this relationship from day one.
